How to submit a delete:
1.
Select
&
Copy
(control
C)
all
text
lines
between
the
two
horizontal
lines
below.
2.
Click
the
e-mail
link
at
the
bottom
of
the
page
to
open
the
e-mail
box.
3.
Paste
(control
V)
the
copied
text
lines
into
the
body
of
the
e-mail
box.
4.
Fill
in
the
blanks
next
to
the
text
items
noted
in
the
e-mail
body.
(all
information
must
be
completed
prior
to a
delete)
(your
name
and
title
must
match
the
name
and
title
on
our
records
to
delete
the
club)
5.
Send
the
completed
e-mail
to us
to
update
the
directory.
